Neurocritical Care
Fellowship Program

Neurocritical Care Fellowship Training at Brunei Neuroscience Stroke and Rehabilitation Centre (BNSRC) for the aspiring Neurocritical Care (Neurointensivist) Specialist.

About

Neurocritical care training in Brunei Darussalam is in its initial stage and is now offered at the Brunei Neuroscience Stroke & Rehabilitation Centre (BNSRC) a specialist center at Jerudong Park Medical Centre (JPMC)  and accredited by the Indian Society of Neuroanaesthesiology and Critical Care (ISNACC). The one year fellowship is offered to fellows who have completed post-graduate fellowship training in anesthesiology, critical care, internal medicine, neurology, and neurosurgery. On completion of the One-year course, an exit exam will be conducted by ISNACC and the successful candidate will receive a Post-Doctoral Fellowship Certificate from ISNACC.

What is Neurocritical Care?

Neurocritical care is the intensive care management of patients with life-threatening neurological and neurosurgical illnesses. Neurocritical care provides the interface between the brain and other organ systems in the setting of critical illness. Patients are taken care of within a single specialised unit. Neurocritical care units specialise in managing the unique needs of such critically ill patients.

Care by Neurointensivists

A neurointensivist is a physician who cares for patients in a Neurocritical Care Unit (NCCU), also known as a Neurological ICU.

The neurointensivists assume the lead role for care of patients in the NCCU, coordinating neurological and medical management. The neurointensivists work collaboratively with a multi-disciplinary team of neurologists, neurosurgeons, neuro-endovascular radiologists, anesthesiologists, critical care nurses, pharmacists, when creating and implementing individualised treatment plans.

Research has shown that specialised care in an NCCU led by a neuro-intensivist is associated with improved outcomes, lower mortality rates, shorter length of stay and lower cost of care compared with a general ICU setting.

Serious neurological injury or illness is often life-changing for the patient and the family. While the neurointensivists and others in the NCCU care team will guide the treatment process, the expanded team of physical therapists, occupational therapists, speech-language pathologists, nutritionists, and social workers, will help in improving outcomes, lower mortality rates, reduce length of stay and lower cost of care.

The availability of organised neurocritical care teams with fellowship-trained neurointensivists, neurocritical care nurses and pharmacists, is limited. To overcome this gap and to upscale the neurocritical care services in Brunei, Brunei Neuroscience Stroke and Rehabilitation Centre has initiated a One-year Fellowship training in Neurocritical Care. The aim is to provide level NCCU which would be a receiving center for patients with the most complex neurological catastrophes who require the most comprehensive neurocritical care.
