Accessing YourMedical Records
The Health Information Management (HIM) Department maintains the medical records of patients who have received care at Jerudong Park Medical Centre (JPMC).
The Release of Information (ROI) Unit manages requests for medical records, medical reports, and related healthcare information. All requests are processed securely and confidentially in accordance with hospital policies and applicable laws and regulations.
Patients may request access to their medical records in accordance with hospital policies and applicable laws and regulations.
Services Available
The ROI Unit can assist with:
- Medical reports
- Insurance form completion
- Copies of medical records and clinical documents
Please note that certain reports or results may only be released following review by your treating doctor.
Required Documents
For Patients or Authorised Representatives
- Completed Application for Release of Medical Information Form
- Copy of the patient’s identification document (NRIC or passport)
- Authorisation Form, if applicable
